Indonesia

Visa on arrival, 30 days. Two separate entries here — Oct 6 and again Oct 30 — so expect to pay it twice. The e-customs declaration also has to be filed fresh for the second entry, within three days of landing.

Nepal

Visa on arrival at Kathmandu. Bring passport photos and USD cash; the kiosks are unreliable.

Bhutan

Not a visa on arrival. It has to be approved in advance through a licensed operator, along with the Sustainable Development Fee for each of the four nights. This is the one entry requirement the original document doesn't mention.
